7/1-7/3 ; 7/5; 7/8-7/12; 7/15-7/19; 7/22-7/26; 7/29-8/2; 8/5-8/9; 8/12-16 ;8/19-8/23; 8/26-8/30; 9/3-9/6; 9/9-9/13; 9/16-9/20; 9/23-9/27

7:00 am-4:30 pm, with 1 hr unpaid lunch

JOB REQUIREMENTS
• Minimum three (3) years primary care practice experience
• Ability to work independently
• Working with an Electronic Medical Record (EMR)
• Experience working with pediatric patients in some locations
• Certain clients and health centers may require phlebotomy

Certificates, Licenses, Registrations:
• Active and unrestricted applicable licensure
• Current CPR (BLS) certification or must be obtained within 60 days of hire
• NPI & DEA
• Current National Board Certification

GENERAL PURPOSE OF JOB This position will focus on prevention, wellness, and maintenance of health in partnership with patients. This will be accomplished by evaluating, treating, and educating patients in a clinic setting. Under the collaboration of a physician, performs professional medical work in the examination, diagnosis, and treatment of patients according to statutory limitations. Examines and diagnoses patients and develops treatment plans. Performs testing, refers to physicians as needed, writes prescriptions, and performs administrative procedures within their scope and licensure of expertise, protocols, and available equipment.

Privileges: Approximately 30 days and could happen quicker. Credentialing goes thru 3rd party vendor who emails provider directly on app/forms, Will need BG, UDS, TB test results, Hep B, BLS from AHA, RedCross or ProCPR, COI. Must clear WI Care Giver Background Check.

**COVID vaccination req'd for coverage unless documented religious or health reasons to "opt out"

**Training occurs the week prior to start date. It is typically 1-2 days and done remotely with a clinical trainer. The training time is paid. (5-6 hrs on-line and then 4 scheduled hours with trainer)
