Non-profit community hospital practice in Northwestern Ohio is seeking an experienced Family Medicine Nurse Practitioner/Physician Assistant for a full-time Primary Care Opportunity located in a community health center.

Job Description:

  • Outpatient only
  • Adults 18+ years
  • Flexible days and hours
  • Light call from home, shared 1:3
  • Avg of 25 patients/day
  • Cerner EMR
  • Practice in a health center that provides family practice and pediatric care to area residents, regardless of ability to pay. For patient convenience there is an on site laboratory, pharmacy assistance program, social workers and a care navigator. All fees are based on a sliding scale, and most insurances are accepted including Ohio Medicaid and self-pay. Financial assistance available for those who qualify. Fees are based on ability to pay.

Compensation and Benefits:

  • Base salary plus w/RVU -- competitive salary range based on experience and training (to be discussed at interview).
  • 200 hours of vacation/CME, 6 days paid holidays, birthday pay, sick leave accrual up to 600 hours
  • Medical, vision and dental insurance, life and long-term disability insurance, malpractice insurance
  • 403 (b) tax deferred annuity, 457 (b) deferred compensation retirement plan
